Bic 100 Children’s Services Referral Form Guidance
Referral for children in need of help, support and protection
This form is only for professionals. If you are a member of the public please call 0300 300 8585
A multi agency referral should be made when an agency considers a child's needs cannot be met solely by that agency and co-ordinated intervention is required to promote, safeguard or protect the welfare of the child/children concerned. Please complete the form as fully as possible. If you require support then please contact the safeguarding officer in your organisation or the Access and Referral Hub.
If a child or young person is at risk of serious harm contact the Access and Referral Hub:

If you have concerns that a child or young person is at risk of serious harm outside of the above hours then please contact the Emergency Duty Team on 0300 300 8123 and the Police.

All telephone referrals must be followed up in writing by the referrer within 48 hours.

- Please include ALL relevant information (including info share on telephone) and complete ALL sections
- If you don't know, please say so.
- Please complete the form as fully as possible to enable the Access and Referral Hub staff to make informed decisions regarding the next course of action.
-The form could be returned to referrer if insufficient information is provided.
Please be aware you cannot save this referral or access it once you have submitted it, however a copy of the completed referral will be emailed to you following your submission
